Supporting Documents Needed

You will need the following documentation when applying for a U.S. passport book or passport card (examples of these documents are found below):

  • Citizen evidence
  • Identification
  • Proof of relationship (for children under 16)

 

The specific documents you need depends on what form you are using.  When in doubt, refer to the form you are filling out as each form identifies necessary documentation.

Most commonly used documentation includes:

  • Citizenship 
    • A U.S. birth certificate (it must be certified)
    • A full-validity U.S. passport book or passport card (a previously issued passport book or card...can be expired, but should not be damaged)  

  • Identification
    • Valid driver's license (not temporary or learner's permit)
    • Previous U.S. passport book or passport card
    • Certificate of Naturalization with identifiable photo attached
    • Certificate of Citizenship with identifiable photo attached
    • Official U.S. military or military dependent identification card
    • Government employee identification card (federal, state, county, municipal)
    • Current (valid) foreign passport

 

  • Proof of relationship (for children under 16)...Note: parents must be able to establish their own identity with documentation listed above.
    • Certified U.S. birth certificate with parents' names
    • Certified foreign birth certificate with parents' names
    • Form FS-240, Consular Report of Birth Abroad, with parents' names
    • Form DS-1350, Certification of a Report of Birth Abroad, with parents' names
    • Adoption decree with adopting parents' names
    • Court order establishing custody
    • Court order establishing guardianship

  • Identity documents for Minors ages 16 and 17
    • Valid driver's license
    • If minor does not have sufficient identification, parent(s) may identify them.  This is done by having the parent submit his/her identification and both the parent and the minor must sign the application in the presence of the Acceptance Agent.
